Description
Running as a slender strap along the medial thigh, the gracilis lies superficial to adductor magnus and posterior to adductor longus, descending from the inferior pubic ramus toward the medial tibial condyle. Distally, its tendon passes posterior to the medial femoral epicondyle and inserts on the anteromedial proximal tibia as part of the pes anserinus, alongside sartorius (anterior) and semitendinosus (posterior). The femur, knee joint, tibia, and fibula provide bony landmarks for its long course, with the gracilis positioned medial to the femoral shaft and then curving anteromedially at the knee. Thin. Superficial. Easy to trace. Clinically, this anatomy matters at the medial knee, where pes anserine bursitis and tendinopathy can mimic medial meniscal pain, and palpation-guided injection relies on knowing the gracilis tendon’s relationship to the tibial tuberosity and medial collateral ligament. Orthopedic and sports medicine teams also exploit the gracilis as a donor tendon for ACL reconstruction, often harvesting it with semitendinosus, so a clear depiction of its distal tendon and pes anserinus footprint supports preoperative teaching and postoperative rehab discussions. Proximally, the muscle’s obturator nerve innervation and role in hip adduction make it a frequent participant in adductor strains, with pain referred along the medial thigh.
