Description

Superior in the anterior neck, the U-shaped hyoid bone sits just inferior to the mandible and tongue base and superior to the thyroid cartilage, suspended in the suprahyoid and infrahyoid musculature rather than articulating with adjacent bones. Its greater horns project posterolaterally, while the lesser horns rise superiorly at the junction of body and cornua, adjacent to the thyrohyoid membrane that spans toward the laryngeal framework. Immediately inferior, the thyroid cartilage forms the anterior laryngeal prominence, with the cricoid cartilage encircling the airway below and the tracheal rings continuing inferiorly. A leaf-shaped epiglottis lies superior and posterior to the thyroid cartilage, related to the hyoepiglottic ligament and the pre-epiglottic space. Orientation of the hyoid relative to the epiglottis and thyroid cartilage is a key teaching point for swallowing mechanics and airway protection, because hyoid elevation and anterior excursion help invert the epiglottis and open the upper esophageal sphincter. Small shifts matter. In trauma and forensics, hyoid and laryngeal cartilage fractures are evaluated in strangulation and hanging, and the greater cornua provide common fracture sites with age-related ossification patterns that influence imaging interpretation. Clinically, the hyoid also serves as a landmark in tracheostomy planning and in assessing high-riding hyoid or laryngeal elevation in dysphagia workups.
