Description
Centered in the posterior neck, the hyoid bone is rendered from behind with the body (corpus ossis hyoidei) forming a gentle posterior convexity and the paired greater horns (cornua majora) sweeping laterally toward their distal tips. Superior to the body, the lesser horns (cornua minora) project as small conical processes near the junction of the body and greater horns, a key attachment point for the stylohyoid ligament. No true articulation is present with other bones; instead, surrounding fascia and suprahyoid and infrahyoid muscular attachments suspend the hyoid in the visceral compartment, just superior to the thyroid cartilage and anterior to the hypopharynx. A floating landmark. Posterior orientation matters because it clarifies the relationship of the hyoid to the pharyngeal wall and to the laryngeal inlet during swallowing, when the hyoid elevates and moves anteriorly to help protect the airway. Surgical and procedural work in the upper airway often references the hyoid as a stable landmark, from open approaches for hypopharyngeal and laryngeal exposure to sleep surgery (for example, hyoid suspension) in obstructive sleep apnea. Forensic evaluation also relies on the cornua, where fractures can occur in manual strangulation, more often with ossified synchondroses in older individuals but still relevant in adults.
