Inferior Epigastric Artery

Description

Emerging from the external iliac artery just superior to the inguinal ligament, the inferior epigastric artery ascends on the deep surface of the anterior abdominal wall toward the rectus sheath. Its course tracks superiorly and medially, passing deep to rectus abdominis as it enters the sheath near the arcuate line, where the posterior layer becomes deficient inferiorly. Surrounding landmarks in this anterior male torso view include the rectus abdominis with its tendinous intersections, the aponeuroses of external and internal oblique, and proximal thigh vessels continuing as the femoral artery and vein inferior to the inguinal ligament. Color separation distinguishes arterial and venous channels within the lower abdominal and upper leg vascular field. For groin and lower abdominal surgery, this artery is a practical hazard and a useful landmark. Its relationship to the deep inguinal ring and the lateral umbilical fold helps distinguish indirect from direct inguinal hernias during open repair and on laparoscopic views of the preperitoneal space. Bleeding from the inferior epigastric artery or its branches is a recognized complication of trocar placement, paracentesis, and lower abdominal incisions, and the vessel also supplies the rectus abdominis muscle used in TRAM and DIEP flap breast reconstruction planning.
