Description

Anteriorly, the inferior epigastric artery is traced as it arises from the external iliac artery just superior to the inguinal ligament and then courses superomedially along the posterior surface of the rectus abdominis within the rectus sheath. The vessel sits deep to the rectus muscle and anterior to the transversalis fascia and peritoneum, with paired venae comitantes accompanying it on either side. Superiorly, it approaches the umbilical region and contributes to an anastomotic network with the superior epigastric artery, a relationship often appreciated when the thoracoabdominal wall vasculature is shown against the rib cage and anterior pelvis. Key landmarks read cleanly in this orientation. For abdominal and pelvic surgeons, this artery is a do-not-hit structure. Its position relative to the deep inguinal ring makes it the classic divider between indirect and direct inguinal hernias, and it is the vessel most often injured during trocar placement in laparoscopic surgery when ports are placed too close to the lateral edge of the rectus muscle. Bleeding from inferior epigastric injury can track into the rectus sheath, producing a rectus sheath hematoma that may mimic an acute abdomen. Reliable surface projection. The course also underpins flap planning for a deep inferior epigastric perforator (DIEP) or TRAM reconstruction, where perforators traverse the rectus to supply the lower anterior abdominal wall.
