Description
Arising from the anterior aspect of the abdominal aorta at approximately the L3 level, the inferior mesenteric artery descends inferiorly and leftward into the mesentery to reach the hindgut. Its typical branches are visualized in relation to the distal transverse colon and splenic flexure (via the left colic artery), the descending and sigmoid colon (sigmoid arteries), and the superior rectum (superior rectal artery), with the vessel course tracking anterior to the lumbar vertebral bodies. Nearby vascular landmarks include the inferior vena cava to the patient’s right and the aortic bifurcation inferiorly, while autonomic nerves of the inferior mesenteric plexus and paraaortic lymphatic channels run in close association with the artery and its branches. Color coding separates arterial, venous, neural, and lymphatic structures. An anterior perspective on the IMA matters when teaching the watershed zones of colonic perfusion, where marginal artery continuity can be variable at Griffiths point near the splenic flexure and at Sudeck’s point near the rectosigmoid junction. Those junctions become clinically concrete during aortic surgery, left colectomy, and sigmoid resection, where ligation level (high tie at the IMA origin versus low tie distal to the left colic artery) influences perfusion of the remaining colon and anastomotic leak risk.
