Description
Framed from the lateral aspect of the male skull and upper cervical spine, the lateral temporomandibular ligament (temporomandibular ligament, lateral ligament) is seen spanning from the articular tubercle and root of the zygomatic arch of the temporal bone to the posterolateral surface of the neck of the mandible, adjacent to the lateral pole of the mandibular condyle. Superior to the attachment lie the squamous temporal bone and zygomatic arch, while the mandibular ramus descends inferiorly toward the angle of the mandible. Posteriorly, the mastoid process and occipital region provide bony context, and the cervical vertebrae align inferior to the cranial base. Orientation matters here because the lateral temporomandibular ligament is the main capsular thickening that limits posterior displacement of the mandibular condyle and resists excessive rotation during opening. In clinical TMJ dysfunction and post-traumatic malocclusion, this lateral restraint helps explain why condylar translation typically occurs anteroinferiorly onto the articular eminence rather than posteriorly into the retrodiscal tissues, a pattern relevant to disc displacement and joint clicking. It also guides reduction mechanics and surgical planning, since lateral approaches to the TMJ must respect the ligament’s course while navigating the preauricular region near the external acoustic meatus and parotid fascia.
