Description

Viewed from inferior with the cranial base oriented superiorly, the mandible frames the floor of the oral cavity with its body and rami while the mandibular nerve (V3) is traced in blue from the middle cranial fossa into the infratemporal region. At the skull base, V3 exits the foramen ovale of the sphenoid, then descends medial to the mandibular ramus toward the region of the lingula and mandibular foramen on the medial surface. Along the inner aspect of the mandibular body, the submandibular fossa is apparent inferior to the mylohyoid line, a concavity that lies medial to the mandibular corpus and relates to the submandibular gland. Orientation matters. This underside perspective is a workhorse for teaching the anatomy that underpins inferior alveolar nerve block technique: the target lies at the mandibular foramen, just superior to the occlusal plane and medial to the ramus, where V3 gives rise to the inferior alveolar nerve before entering the mandibular canal. It also helps explain referred otalgia and temporomandibular pain patterns, since V3 carries sensory fibers from the TMJ and adjacent dura and motor fibers to the muscles of mastication, which occupy the infratemporal fossa inferior to the cranial base. Dental anesthesia modules, head and neck anatomy courses, and oral and maxillofacial surgery texts commonly need this specific relationship between foramen ovale, infratemporal fossa, and mandibular landmarks in a single frame.
