Description
Posterior right-sided anatomy of the upper cervical region is presented with the medial semispinalis capitis traced deep to the superficial posterior neck layer, running superomedially from the cervical and upper thoracic spinous and transverse processes toward the occipital bone. Its fibers lie medial to splenius capitis and splenius cervicis, with the muscle belly hugging the midline over the laminae and spinous processes of the upper cervical vertebrae. Superiorly, its insertion approaches the nuchal lines of the occiput just inferior to the posterior calvaria, while inferiorly it blends into the transversospinalis mass over C3 to T6. Bony landmarks include the occipital bone, cervical vertebrae, scapula, clavicle, proximal humerus, and upper ribs. Deep, midline, and easy to underestimate. Semispinalis capitis drives cervical extension and contralateral rotation, so a posterior view with the head turned left helps explain how the right medial fibers contribute to rotation control while stabilizing the upper cervical segments. In whiplash-associated disorders and chronic neck pain, myofascial trigger points in semispinalis capitis can refer pain to the occiput and vertex and mimic cervicogenic headache patterns. The muscle’s close relationship to the C2 dorsal ramus and the greater occipital nerve makes it relevant when planning occipital nerve blocks, decompression, or when interpreting tenderness and paresthesia along the posterior scalp.
