Description
Running obliquely across the posterolateral neck, the splenius capitis lies deep to the upper trapezius and superficial to semispinalis capitis, its fibers sweeping superolaterally from the lower cervical and upper thoracic midline toward the mastoid process and lateral third of the superior nuchal line. Medially, the paired splenii frame the nuchal ligament and the spinous processes around C7 to T3 or T4, while laterally they blend toward the posterior border of the sternocleidomastoid region at the skull base. Inferiorly, the muscle belly transitions toward the cervicothoracic junction, just superior to the broad sheet of latissimus dorsi and the longitudinal columns of erector spinae. Palpation and injection planning depend on this posterior relationship, because the splenius capitis commonly harbors trigger points that refer pain to the occiput and vertex and can mimic cervicogenic headache. Clinically, this view also clarifies why whiplash and repetitive extension rotation strain tends to localize along the posterolateral cervical region, where splenius capitis works with the ipsilateral obliquus capitis inferior to rotate the head and with bilateral contraction to extend the cervical spine.
