Description
Prominent in the posterior neck, the paired splenius capitis muscles form a broad, superficial V as they converge toward the midline and sweep superolaterally to the skull. Each muscle arises from the nuchal ligament and spinous processes of C7 to T3 or T4, then courses obliquely to insert on the mastoid process of the temporal bone and the lateral third of the superior nuchal line of the occipital bone. Deep to this layer, the semispinalis capitis typically appears as a more vertical, paramedian column, with the occiput superiorly and the cervical spinous processes inferiorly providing clear bony reference points. Scapulae and upper thoracic elements sit laterally and inferiorly at the base of the field. Posterior layering matters in the neck. Splenius capitis is a common target when teaching regional anatomy because it sits at the interface between superficial trapezius coverage and the deeper transversospinalis group, and its fiber direction explains ipsilateral rotation with simultaneous extension.
