Description
Seen in lateral profile, the splenius capitis forms a broad, strap-like layer along the posterolateral neck, running superolaterally from the midline ligamentum nuchae and upper thoracic spinous processes toward the mastoid process and the lateral third of the superior nuchal line. Anterior to it, the sternocleidomastoid spans from the manubrium and clavicle to the mastoid, creating a clear anterior boundary of the cervical region. Deep and medial, the cervical vertebrae align beneath the occipital bone, while the mandible and dentition sit anteriorly under the cranial vault. Bone anchors matter here. Lateral orientation clarifies the opposing roles of splenius capitis and sternocleidomastoid in head posture: splenius capitis extends and ipsilaterally rotates the head, while sternocleidomastoid tends to flex the neck and rotate the face contralaterally. That relationship is a practical teaching point when sorting muscular causes of limited rotation after whiplash or prolonged forward-head posture, where posterior cervical myofascial pain often localizes near the mastoid insertion.
