Description
Running along the anterior aspect of the cervical spine, the middle part of the longus colli is shown as a paired, strap-like deep neck flexor spanning the lower cervical vertebral bodies and adjacent transverse processes. Its muscle belly lies immediately anterior to the vertebral column and medial to the scalene region, with the carotid sheath structures positioned more laterally in the neck. Superiorly it approaches the upper cervical segments beneath the skull base, while inferiorly it continues toward the cervicothoracic junction, deep to the visceral compartment of the neck. Longus colli matters because it is the muscular bed you meet when you approach the anterior cervical spine, and its surface anatomy helps orient level localization during anterior cervical discectomy and fusion (ACDF). Retraction or disruption of the longus colli and its investing fascia is also tied to postoperative prevertebral edema and contributes to dysphagia, so clear depiction of its mid-cervical portion supports both planning and counseling.
