Description
Running vertically on the anterior aspect of the cervical spine, the middle (straight) fibers of the longus colli lie deep to the pharynx and esophagus and immediately anterior to the vertebral bodies and intervertebral discs. From a lateral profile, the muscle tracks parallel to the anterior longitudinal ligament, spanning the mid to lower cervical levels and approaching the cervicothoracic junction superior to the first ribs. Posterolateral to it sit the transverse processes and the articular pillars, while more superficially the hyoid, mandible, and the contour of the sternocleidomastoid region establish the anterior neck silhouette. This lateral presentation matters because the longus colli defines the prevertebral layer of deep cervical fascia, a key surgical plane in the standard anterior cervical approach for discectomy, fusion, and corpectomy, where retraction must respect the carotid sheath laterally and the tracheoesophageal complex medially. It is also the muscle implicated in acute calcific tendinitis of the longus colli, a cause of severe neck pain and odynophagia that can mimic retropharyngeal abscess on CT by producing prevertebral soft-tissue swelling and effusion. Seeing its vertical belly in relation to the vertebral bodies helps explain both exposure strategy and imaging findings.
