Description
Running along the posterior cervical midline, the nuchal ligament (ligamentum nuchae) is rendered as a thickened, septal continuation of the supraspinous ligament, extending from the external occipital protuberance and median nuchal line inferiorly to the spinous process of C7. A lateral skeletal view places the occipital bone superior and posterior to the cervical vertebrae (C1 to C7), with the mandible and hyoid bone lying anterior to the cervical column. Subtle relationships to the posterior elements of the vertebrae are implied, with the ligament positioned dorsal to the vertebral bodies and intervertebral discs and spanning the interspinous intervals. Clear midline anatomy. Clinically, this structure matters because it is a palpable and surgical landmark in the posterior neck, marking the median plane between paired paraspinal muscles while providing attachment for trapezius, splenius capitis, and other posterior cervical musculature. During posterior cervical exposure for laminectomy, laminoplasty, or instrumented fusion, midline dissection commonly follows the nuchal ligament to reduce muscular trauma and blood loss. Degenerative cervical spondylosis and trauma can also alter the posterior tension band, and appreciation of the ligament’s occiput to C7 continuity helps explain why flexion injuries may present with midline posterior neck pain and tenderness near the C7 prominence. Neuroanatomy and musculoskeletal anatomy courses can use this plate to teach posterior cervical surface landmarks, the relationship of the skull base to the cervical spine, and why C7 serves as a common counting point on physical exam and imaging. It also fits well in spine surgery atlases, procedural patient education on posterior cervical approaches, and radiology teaching files where you want to correlate midline soft-tissue anatomy with lateral cervical radiographs or sagittal CT/MRI reconstructions.
