Description
Posterolateral head and neck anatomy is presented with the occipital scalp and posterior face overlying the cranium and upper cervical region, then descending into the cervical soft tissues superficial to the cervical vertebrae. Along the lateral neck, the sternocleidomastoid forms an oblique boundary between the anterior triangle and posterior triangle, while the trapezius sweeps inferolaterally toward the shoulder; between them, you would expect the splenius capitis and levator scapulae to appear deeper and more medial as they approach the mastoid process and transverse processes. Red musculature is interlaced with arterial and venous channels (red and blue) and superficial sensory and motor nerves (yellow) as they course superiorly toward the scalp and auricular region and inferiorly toward the supraclavicular fossa. A posterolateral perspective matters because it clarifies relationships that drive common clinical problems: the spinal accessory nerve (CN XI) traverses the posterior triangle after innervating sternocleidomastoid, then runs on the deep surface of trapezius, a pattern that explains postoperative shoulder droop after posterior triangle lymph node biopsy. Palpating the carotid pulse, placing central venous access, or planning a neck dissection all depend on a clean mental map of where the sternocleidomastoid sits relative to the carotid sheath and superficial venous drainage, even when only surface landmarks are available. Small structures, big consequences.
