Description
Presented in lateral profile, the occipital bone and upper cervical spine frame the posterior atlantooccipital membrane as it spans from the posterior margin of the foramen magnum to the posterior arch (lamina) of the atlas (C1). The membrane forms a thin fibrous sheet along the dorsal boundary of the vertebral canal entrance, lying posterior to the dural sac and deep to the suboccipital musculature. Immediately inferior, the posterior elements of the axis (C2) align with C1, establishing the craniocervical junction in anatomical position. Skull landmarks including the zygomatic arch, mandible, and dentition provide facial orientation. Clinically, this structure matters because it bridges the cranial base to C1 and converts the posterior atlantooccipital notch into the arcuate foramen when ossified, a variant that can alter the course of the vertebral artery and the suboccipital nerve. That relationship becomes relevant in posterior C1 screw placement, suboccipital decompression, and when interpreting cervicogenic headache syndromes or vertebrobasilar insufficiency related to arterial tethering at the craniovertebral junction.
