Description
Centered on the posterior cerebral artery (PCA), the illustration traces its origin from the basilar artery bifurcation at the superior border of the pons and its posterior course as it curves laterally around the midbrain within the ambient cistern. Proximally, the P1 segment runs medial to the cerebral peduncle toward the posterior communicating artery connection, then continues as P2 toward the tentorial edge and the medial surface of the temporal and occipital lobes. A semi transparent calvaria reveals vascular impressions along the internal table, with arterial channels rendered in red and accompanying venous pathways in blue for orientation. Dentition of the mandible remains visible inferiorly, anchoring the cranial base in anatomical position. Course and neighbors matter with the PCA. Infarction in the PCA territory commonly produces contralateral homonymous hemianopia from occipital cortex involvement, and thalamic perforators arising near P1 can add sensory syndromes or altered arousal when compromised. Aneurysms at the basilar tip or along the P1 to posterior communicating junction sit adjacent to the oculomotor nerve in the interpeduncular cistern, so a third nerve palsy is a real bedside clue when subarachnoid hemorrhage is suspected. The curved sweep of the artery around the midbrain also clarifies why tentorial herniation and posterior fossa masses can distort this circulation. Neuroanatomy and neurosurgery teaching teams will find this angle useful for correlating cisternal anatomy with CTA, MRA, and catheter angiography, and for explaining the segmentation of the PCA (P1 to P4) in stroke localization.
