Description

Arising from the intracranial segment of the vertebral artery near the foramen magnum, the posterior meningeal branch courses superiorly and posteriorly along the inner surface of the occipital bone to supply the dura mater of the posterior cranial fossa. A calvarial cutaway exposes the endocranial base, with the arterial tree traced across the cranial cavity in relation to the margins of the foramen magnum and the posterior clinoid region. Smaller arteriolar branches can be appreciated spreading laterally toward the dural reflections and adjacent venous sinus grooves. Spatial context matters. Attention to this vessel pays off when teaching posterior fossa dural vascular anatomy and when planning approaches around the craniocervical junction. The posterior meningeal branch is a recognized contributor to dural arteriovenous fistulas supplied by the vertebral system, and its proximity to the vertebral artery at the foramen magnum makes it relevant during suboccipital craniotomy, far-lateral approaches, and C1 laminectomy where unintended dural bleeding can complicate exposure. Its relationships also help clarify how meningeal supply in the posterior fossa differs from the middle meningeal artery dominant pattern over the calvaria.
