Superficial Temporal Vein

Description

Running superior to the auricle, the superficial temporal vein ascends within the temporal scalp and courses over the temporalis region toward the zygomatic arch, where it continues inferiorly to contribute to the retromandibular vein within or deep to the parotid gland. Anterior and posterior tributaries from the lateral scalp converge along the lateral cranial vault, lying superficial to the temporal fascia and in close relationship to the superficial temporal artery. The pinna, external acoustic meatus, zygomatic arch, and portions of the temporal bone provide bony landmarks, while adjacent musculature such as the temporalis and sternocleidomastoid frames the lateral head and upper neck. Palpable and often visible in lean patients, this vessel matters when planning temporal scalp incisions, endoscopic brow lift portals, or parotid approaches where venous bleeding can obscure the operative field. Its connections to the retromandibular system also explain why parotid inflammation or surgery can produce troublesome venous congestion and bruising along the preauricular and temporal skin. Venous access and complications belong here too, since superficial temporal vein cannulation is sometimes used in pediatric or difficult-access patients, and inadvertent injury can create a pulsatile-appearing hematoma when the neighboring superficial temporal artery is involved.
