Description
Centered on the anterior knee, the patella sits anterior to the distal femur and continues inferiorly as the patellar ligament (patellar tendon), which normally spans from the patellar apex to the tibial tuberosity on the proximal tibia. A full-thickness tear interrupts that longitudinal collagen bundle, leaving a conspicuous gap between the proximal and distal stumps with frayed ends. Medial and lateral tibial condyles frame the proximal tibial plateau, and the fibular head lies posterolateral to the tibia as a bony landmark for lateral-sided anatomy. The extensor mechanism is disrupted. Patellar ligament rupture is a classic extensor mechanism injury, most often after a forceful eccentric quadriceps contraction during landing or sudden deceleration, or in patients with tendinopathy, corticosteroid exposure, or systemic disease. Clinically it presents with loss of active knee extension and a high-riding patella (patella alta) on lateral radiographs because the quadriceps tendon retracts the patella proximally when the distal tether to the tibial tuberosity fails. Surgical repair targets end-to-end approximation or transosseous fixation, and this tear morphology helps explain why early reattachment and protected range of motion matter for restoring the extensor lever arm. It also helps distinguish anterior extensor failure from intra-articular instability patterns such as anterior cruciate ligament injury, where the primary complaint is giving way rather than inability to perform a straight-leg raise.
