Description

Anterior knee anatomy is centered on the patella and the patellar ligament (patellar tendon) spanning inferiorly to the tibial tuberosity, with the quadriceps tendon inserting on the superior pole of the patella and blending into the medial and lateral patellar retinacula. A focal rupture is represented by discontinuous collagen fibers near the tibial tuberosity, creating a gap in the normally taut band that lies superficial to the infrapatellar (Hoffa) fat pad and anterior to the tibial plateau. Laterally, the proximal tibia leads toward the fibular head, a useful landmark for orientation when comparing medial and lateral soft tissue contours. Skin creases and the prepatellar region frame the extensor mechanism. Patellar ligament tears matter because they interrupt the knee extensor mechanism and produce an immediate functional deficit, classically inability to perform an active straight leg raise, often accompanied by patella alta when the ligament fails distally at or near the tibial tuberosity. Location is not academic: distal avulsions and midsubstance ruptures demand different fixation strategies, and the relationship to the tibial tubercle guides surgical exposure while protecting the infrapatellar branch of the saphenous nerve and the anterior tibial periosteum. Expect to correlate this anatomy with sports-related eccentric loading injuries, chronic tendinopathy, and postoperative complications after tibial tubercle procedures.
