Description
Seen from a posterior torso orientation, the transversus abdominis forms a broad, muscular sheet wrapping circumferentially around the anterolateral abdominal wall, deep to the external oblique and internal oblique. Its fibers run predominantly horizontally from the thoracolumbar fascia, iliac crest, and costal cartilages toward the linea alba and pubic crest, creating a belt-like arrangement inferior to the rib cage and superior to the pelvis. Along the midline, the spinous processes of the thoracic and lumbar vertebrae align superior to the sacrum, while the paired iliac crests and posterior iliac spines bracket the lower attachment field. Ribs arc laterally from the vertebral column, marking the superior boundary of the abdominal wall musculature. Clinically, this view clarifies the transversus abdominis as the deepest of the anterolateral abdominal muscles and a primary contributor to dynamic abdominal pressurization when coupled with the diaphragm and pelvic floor. That relationship matters when teaching core stabilization, managing low back pain, or explaining why poor recruitment can increase strain on the lumbar spine through the thoracolumbar fascia.
