Rendered in transverse section, the myelon occupies the center with a butterfly-shaped grey matter core surrounded by peripheral white matter, while a circumferential venous network courses along the pial surface. Radial intramedullary venules track inward from the periphery toward the central regions, converging near the anterior median fissure and posterior median sulcus in patterns consistent with the anterior and posterior spinal venous drainage. Longitudinal channels align superior to inferior along the cord’s surface, interconnected by bridging veins that communicate with the perimedullary plexus. Venous outflow from the spinal cord is the part many learners gloss over, yet it becomes clinically concrete in conditions like spinal dural arteriovenous fistula, where arterialized perimedullary veins and congested intramedullary venules drive progressive myelopathy.
