Description
Presented as a transverse section of the myelon, the beige spinal cord segment centers the gray matter (anterior and posterior horns) within surrounding white matter funiculi, with the central canal near the midline and the anterior median fissure opposed by the posterior median sulcus. Blue venous channels and venules course over the pial surface and toward the periphery, consistent with the intrinsic spinal cord veins draining into the coronal venous plexus and then into the epidural internal vertebral venous plexus within the vertebral canal. Paired spinal nerves emerge bilaterally, with dorsal (posterior) and ventral (anterior) rootlets converging toward a mixed spinal nerve lateral to the cord. This sectional perspective matters because spinal venous drainage is valveless and communicates freely with segmental veins and the vertebral venous plexuses, creating a low pressure pathway for hematogenous spread of infection and metastases into the spine and spinal canal. It is also the anatomy behind venous congestion in spinal dural arteriovenous fistula, where impaired outflow raises venous pressure and produces progressive myelopathy, often with long segment T2 cord hyperintensity on MRI.
