Seen from the dorsal (superior) aspect, the male foot is opened to the subcutaneous layer so the dorsum of the tarsus and metatarsus reads clearly beneath a thin sheet of intrinsic muscle. The extensor digitorum brevis spreads distally from the anterior calcaneus to the dorsal bases of toes two through four, while extensor hallucis brevis tracks anteromedially toward the proximal phalanx of the hallux, both lying superficial to the cuneiforms, cuboid, and proximal metatarsals. Under and between these soft tissues, the talus and calcaneus sit posteriorly, the navicular lies medial to the cuboid, and the five metatarsals project distally toward the phalanges with the metatarsophalangeal joints aligned just proximal to the toes. Ligamentous bands and dorsal joint capsules rim the tarsometatarsal and metatarsophalangeal articulations. Clean landmarks. Dorsal foot anatomy matters because many clinically relevant structures are close to the skin and frequently targeted by trauma, incisions, and injections. The dorsalis pedis artery and deep fibular (peroneal) nerve course over the talonavicular region toward the first intermetatarsal space, and their relationship to the extensor tendons is a constant reference during vascular assessment and dorsal approaches. At the midfoot, the dorsal tarsometatarsal (Lisfranc) complex anchors the metatarsal bases to the cuneiforms; subtle widening between the first and second rays after a twist injury should raise concern for Lisfranc disruption, even when swelling obscures surface landmarks. Use this illustration in lower limb gross anatomy and musculoskeletal courses when teaching intrinsic dorsum musculature versus long extensor tendons, and when orienting students to bony columns of the medial and lateral foot. It also supports orthopedic and podiatric publications describing dorsal incisions for tarsometatarsal fixation, metatarsophalangeal capsulotomy, or dorsal ganglion excision.
