Description
Anterior exposure of a male brachium highlights the humerus from the proximal head and anatomical neck down toward the distal shaft, with the scapula and clavicle partially framed superiorly at the shoulder girdle. A fracture line crosses the proximal humerus at the surgical neck, with visible displacement between the humeral head region and the diaphysis; adjacent soft tissues are opened to keep the cortical margins and fracture gap unobstructed. Distally, the elbow region is only partly included, enough to orient the long axis of the bone and its relationship to the forearm. Red graphic accents mark the injury zone. Surgical neck fractures sit at a clinically unforgiving crossroads because the axillary nerve and posterior circumflex humeral artery course posteriorly through the quadrangular space and wrap around the proximal humerus just inferior to the tubercles. Traction, displacement, or swelling here can present with deltoid weakness, loss of shoulder abduction strength, and sensory change over the lateral shoulder (superior lateral cutaneous nerve of arm) even when the skin findings look modest. Reduction strategy and fixation planning depend on understanding how the proximal fragment aligns relative to the shaft in an anterior view. Neurovascular status matters.
