Presented in anterior aspect, the male humerus is oriented from the rounded proximal head and anatomical neck superiorly to the distal trochlea and capitulum inferiorly, with the shaft centered in the field. The deltoid tuberosity sits on the anterolateral mid-diaphysis, and a clean transverse fracture line crosses the shaft just proximal to that landmark, separating proximal and distal fragments with minimal angulation. Medially, the shaft leads toward the medial epicondyle and supracondylar ridge, while the lateral border tracks toward the lateral epicondyle and the region where the brachioradialis and extensor carpi radialis longus originate. Clean geometry. A subtle perifracture color change suggests acute injury. Transverse humeral shaft fractures most often follow a direct blow and behave differently from the more common torsional spiral patterns, which matters when you are teaching mechanism of injury and planning fixation. Even though the radial nerve runs posteriorly in the spiral groove, a mid-shaft fracture at the level of the deltoid insertion still raises concern for radial nerve palsy and associated weakness of wrist and finger extension, and distal pulses should be checked because the brachial artery becomes more vulnerable as you approach the distal third. For operative discussions, this level is a practical bridge between anterolateral plating corridors and intramedullary nailing, and it frames why fracture location relative to the deltoid tuberosity influences exposure and soft-tissue handling.
