Description
Oriented frontally in anatomical position, the vermiform appendix projects inferiorly from the cecum in the right lower quadrant, with lymphatic vessels running along the appendiceal wall and within the mesoappendix. Small lymph nodes punctuate these channels as they course proximally toward the ileocecal region, where drainage converges along the cecal and ileocolic vascular pedicles. Blue-caliber vessels track in parallel, reflecting the close relationship between lymphatics and the regional venous drainage, while yellow fat outlines the mesenteric folds that carry these structures. Appendiceal lymphatic drainage matters because it predicts how infection and tumor spread beyond the lumen, not just where pain localizes. In acute appendicitis, reactive enlargement of appendicular and ileocolic nodes can mimic or accompany terminal ileitis, and the mesoappendix is the corridor a surgeon controls during appendectomy to ligate the appendiceal artery while avoiding troublesome bleeding from the ileocolic branches. For appendiceal neuroendocrine tumors and adenocarcinoma, nodal basins along the ileocolic vessels guide staging and the decision to extend resection to a right hemicolectomy. A practical roadmap.
