Description

Posterior anatomy of the vermiform appendix is presented with the cecum and adjacent ileocecal region, the appendiceal tip projecting inferiorly from the posteromedial cecal wall. A web of lymphatic vessels courses along the mesoappendix, running proximally toward the ileocolic lymph nodes situated superiorly and medially within the ileocolic mesentery. Small appendiceal lymph nodes cluster near the base of the appendix, with collecting trunks following the vascular arcade toward the cecal and ileocolic nodal groups. Posterior peritoneal relationships are emphasized, clarifying how these channels track deep to the cecum rather than across its anterior surface. This posterior perspective matters when you are thinking about how appendiceal inflammation and neoplasia spread beyond the right lower quadrant. Acute appendicitis can seed bacteria into the mesoappendix and regional nodes, and reactive enlargement of ileocolic nodes can mimic appendicitis clinically and on imaging. In appendiceal carcinoid and adenocarcinoma, nodal drainage along the ileocolic pedicle underpins the oncologic logic of right hemicolectomy with lymphadenectomy, not simple appendectomy, when high risk features are present. The posterior course also explains why retrocecal appendicitis may present with back or flank pain and a relatively quiet anterior peritoneum.
