Description
Seen from the posterior aspect, superficial lymphatic vessels course within the subcutaneous tissues overlying the extensor compartment of the forearm, forming a fine network that converges into longer collectors running proximodistally toward the elbow. Along the dorsum of the hand, channels arise near the metacarpals and phalanges and track proximally across the dorsal wrist toward the posterolateral forearm, where they travel in the same general corridor as superficial veins. Deep to these lymphatics, the extensor muscle bellies and tendons are exposed in red, with blue vascular channels and green cutaneous nerves running in parallel planes. Posterior superficial lymphatic mapping matters when you are trying to explain why dorsal hand infections and lacerations often declare themselves first with linear lymphangitis and tender epitrochlear or axillary nodes rather than deep forearm pain. It is also the anatomic basis for sentinel lymph node pathways after melanoma or squamous cell carcinoma on the dorsum of the hand and posterolateral forearm, where drainage can follow cephalic territory to deltopectoral and apical axillary basins, or pass medially toward the cubital nodes.
