Description
Seen in lateral section, the male larynx is opened to the airway lumen from the laryngeal inlet superiorly to the subglottic cavity and proximal trachea inferiorly. The epiglottis sits anterior to the laryngeal vestibule, while the arytenoid region and posterior laryngeal wall lie posterior, framing the rima glottidis between the right and left vocal folds. Anteriorly, the thyroid cartilage forms the prominent external wall, and the cricoid cartilage creates the complete cartilaginous ring at the inferior margin of the larynx. The true vocal fold is positioned inferior to the vestibular (false) fold, defining the ventricle between them. A lateral cut through the glottis matters because it makes the layered relationships that govern voice and airway protection easy to teach: epiglottic deflection toward the aditus laryngis during swallowing, and vocal fold position during phonation versus respiration. Male laryngeal anatomy is also clinically specific, with a longer membranous vocal fold and larger thyroid angle contributing to lower fundamental frequency, and with the subglottis and cricoid ring representing a fixed-diameter segment that becomes relevant in post-intubation subglottic stenosis. Small distances here have consequences. The proximity of the vocal fold mucosa to the thyroid cartilage and cricothyroid membrane also underpins access routes for emergent cricothyrotomy and for injection laryngoplasty in unilateral vocal fold paralysis.
