Description
Seen from a lateral perspective, the atlas (C1) forms a ring of bone between the occiput and the axis (C2), with the anterior arch positioned anterior to the spinal canal and the posterior arch curving posteriorly to complete the vertebral foramen. Superiorly, the C1 superior articular facet faces upward to receive the occipital condyle, while inferiorly the inferior articular facet meets the superior articular surface of C2. The lateral mass of C1 sits thickest at the midpoint of the ring, and the transverse process projects laterally, marking the route of the vertebral artery through the transverse foramina at adjacent levels. C2 to C7 stack inferiorly with intervertebral discs separating the typical cervical vertebral bodies. Orientation at C1 and C2 matters in a way the rest of the cervical spine does not. Because the atlas lacks a vertebral body, axial load transfers through the lateral masses, and the relationship between the anterior arch of C1 and the dens (odontoid process) of C2 defines the atlanto-odontoid interval, a key measurement when you suspect transverse ligament injury after trauma or in rheumatoid arthritis.
