Description

Prominent in this cervical spine rendering, the atlas (C1) forms a complete osseous ring with paired lateral masses connected by anterior and posterior arches, positioned superior to the axis (C2) and inferior to the occipital condyles. Articular facets on the superior surface of the atlas face superomedially for the atlanto-occipital joints, while the inferior facets face inferomedially toward the superior articular facets of C2. Immediately posterior to the anterior arch sits the dens (odontoid process) of the axis, rising cranially into the atlas ring, and the remaining cervical vertebrae are shown more distally in a neutral tone with intervertebral discs separating the vertebral bodies. At C1 to C2, anatomy and biomechanics meet fast. The dens and the anterior arch of the atlas define the anterior boundary of the spinal canal at this level, where instability can translate into cord compromise, a common concern in odontoid fractures, atlantoaxial subluxation in rheumatoid arthritis, and ligamentous injury after flexion-extension trauma. Surgeons and radiologists also care about the atlas lateral masses as fixation targets and as landmarks when correlating open-mouth odontoid radiographs and CT with suspected Jefferson (burst) fracture patterns.
