Description
Seen from above, the atlas (C1) forms a ring of bone around a wide vertebral foramen, with robust lateral masses positioned posterolateral to the anterior arch and anterolateral to the posterior arch. Each superior articular facet caps its lateral mass and presents a concave, kidney shaped surface oriented superiorly and medially to receive the occipital condyle. Laterally, the transverse processes project from the lateral masses and frame the transverse foramina for the vertebral artery. No vertebral body is present. Attention to the superior articular surface matters because this is the bony half of the atlanto-occipital joint, where flexion and extension of the head (the yes motion) occurs while rotation is largely deferred to the atlanto-axial complex below. Facet curvature and orientation help explain clinical patterns such as occipital condyle fractures, atlanto-occipital dissociation after high energy trauma, and degenerative facet arthropathy that can narrow joint space and alter craniocervical alignment. The broad vertebral foramen and proximity of the transverse foramina also set the stage for understanding why instability at C1 can threaten the spinal cord and why vertebral artery anatomy must be respected during posterior C1 screw placement and lateral mass instrumentation. Use this superior perspective to teach upper cervical osteology in gross anatomy and neuroanatomy courses, or to support figure plates in spine surgery texts discussing craniocervical junction biomechanics and fixation strategies. It also fits radiology education when correlating bony landmarks with CT of the atlas and craniocervical junction trauma classifications.
