Description
Presented is the axis (C2) from an anterior perspective, oriented to emphasize the posterior surface of the dens (odontoid process) within the vertebral canal. The dens projects superiorly from the vertebral body, and its posterior aspect faces the spinal cord, while the anterior surface would normally articulate with the anterior arch of the atlas (C1). Lateral to the body, the transverse processes contain the transverse foramina, and the superior articular facets sit superolaterally, positioned to meet the inferior facets of C1. Attention centers on the roughened posterior dens, the footprint for the transverse ligament of the atlas, which spans between the medial tubercles of C1 and удержs the dens against the anterior arch to form the median atlantoaxial joint. A small structure, big consequences. Failure of this restraint, whether from rheumatoid arthritis, Down syndrome–associated ligamentous laxity, or trauma, permits anterior atlantoaxial subluxation and can narrow the cervical canal, placing the cervicomedullary junction at risk. This angle also helps clarify why a Type II dens fracture at the base of the odontoid is unstable and why reduction maneuvers and screw trajectories must respect the canal immediately posterior to the dens. Use it for teaching upper cervical spine anatomy in gross anatomy, neuroanatomy, and orthopedic or neurosurgical modules that cover atlantoaxial stability, dens fractures, and preoperative planning landmarks. It also suits radiology and spine texts when paired with open-mouth odontoid and sagittal CT/MRI correlates, where the relationship between the dens, vertebral foramen, and transverse foramina is the teaching target.
