Seen from a superior perspective, the atlas (C1 vertebra) forms a broad osseous ring with a short anterior arch and a longer posterior arch enclosing the vertebral foramen. Laterally, the transverse processes project beyond the lateral masses, and the transverse foramina sit at their bases for passage of the vertebral arteries. Anteromedially, the anterior tubercle and the facet for the dens of C2 lie on the inner surface of the anterior arch, while posteriorly the posterior tubercle replaces the spinous process typical of other cervical vertebrae. Concave superior articular facets, capped with articular cartilage pads, face superomedially to receive the occipital condyles at the atlanto-occipital joints. Appreciating the atlas from above helps clarify why C1 fractures behave differently from subaxial cervical injuries: without a vertebral body, axial loading tends to drive the lateral masses outward, producing the classic Jefferson burst pattern and potential widening at the atlantoaxial complex. The relationship between the lateral mass, transverse foramen, and vertebral artery groove matters when evaluating trauma CT or planning posterior C1 lateral mass fixation, where screw trajectory must respect the artery’s course and the spinal canal just medial to the lateral mass.
