Description
Running along the anterolateral compartment of the leg, the extensor digitorum longus arises from the lateral condyle of the tibia and proximal fibula, then narrows into a tendon that passes anterior to the ankle joint beneath the extensor retinacula. Distal to the talus, its slips course dorsally over the lateral aspect of the foot toward the lateral four toes, lying anterior to the lateral malleolus and lateral to tibialis anterior, while remaining medial to fibularis longus and brevis. The lateral foot skeleton is shown in context, including calcaneus, talus, tarsals, metatarsals, and phalanges, with joint lines marking the ankle, subtalar, and midfoot articulations. Clear tendon trajectories. Clinically, this lateral perspective is the one you want when teaching dorsiflexion and toe extension mechanics and how they couple with foot clearance during gait. The extensor digitorum longus becomes a practical landmark in anterior ankle approaches and in assessment of common fibular (peroneal) nerve injury, where weak toe extension and a slapping gait reflect denervation of the anterior compartment. Its course anterior to the ankle also helps explain anterior ankle tendinopathy and the relationship of extensor tendons to retinacular thickening or lacerations over the dorsum of the foot.
