Description

Running along the dorsolateral forearm, the extensor pollicis longus (EPL) muscle belly is shown tapering into its tendon as it courses distally toward the thumb. Proximally, the EPL arises from the posterior surface of the ulna and the interosseous membrane, then travels inferiorly and laterally toward the wrist, where it turns obliquely across the dorsal radius. At the carpus, the tendon passes around Lister’s tubercle (dorsal tubercle of the radius), forming a clear change in direction before continuing to the distal phalanx of the thumb. Bony context commonly includes the humerus at the elbow, the radius laterally and ulna medially, and the carpal bones leading into the first metacarpal and phalanges. That dorsal radial turn is the point. Wrapping around Lister’s tubercle makes the EPL a functional pulley for thumb interphalangeal extension, but it also explains why the tendon is vulnerable to attritional rupture after distal radius fractures or following volar plating when hardware prominence or post-traumatic synovitis irritates the third extensor compartment. Clinically, this relationship underpins bedside assessment such as retropulsion of the thumb and the thumb component of the anatomic snuffbox boundary, where the EPL forms the ulnar border opposite the abductor pollicis longus and extensor pollicis brevis. Orthopedic and hand surgery texts use this perspective to orient learners to the dorsal wrist compartments and to plan approaches that protect the EPL during distal radius fixation or tendon transfer. Gross anatomy and kinesiology courses will also find it well suited for teaching thumb extensor mechanics and tendon routing relative to radius, ulna, and carpal landmarks.
