Description
Running along the lateral forearm beneath the skin, the extensor carpi radialis longus (ECRL) forms a fusiform muscle belly that arises from the distal lateral supracondylar ridge of the humerus and narrows into a long tendon as it approaches the wrist. In lateral profile it lies posterior to the brachioradialis proximally, then becomes more superficial as both muscles track toward the radial styloid, with the ECRL tendon passing dorsal to the wrist under the extensor retinaculum. Distally, its tendon courses toward the base of the second metacarpal, parallel to extensor carpi radialis brevis and lateral to the extensor digitorum tendons, while the radius and ulna provide the bony framework for the elbow and radiocarpal joints. Clinically, this lateral view matters because ECRL contributes to wrist extension and radial deviation and is a frequent donor in tendon transfer planning, where surgeons need a mental map of its length, excursion, and relationship to neighboring compartments. The interval between the first dorsal compartment tendons (abductor pollicis longus and extensor pollicis brevis) and the ECRL/ECRB tendons underlies intersection syndrome, a classic overuse pain generator in rowers and weightlifters that localizes a few centimeters proximal to the wrist. Appreciating how the ECRL runs deep to brachioradialis also supports discussions of superficial radial nerve symptoms, since that nerve emerges from under brachioradialis in the distal forearm near these tendon corridors.
