Description
Running along the radial side of the forearm, the extensor carpi radialis longus (ECRL) lies in the superficial extensor compartment with its fusiform belly proximal and its tendon narrowing as it courses distally toward the dorsum of the wrist. From a medial perspective with the skin removed, the brachioradialis sits immediately adjacent, and the ECRL can be followed from its origin on the lateral supracondylar ridge of the humerus toward the distal radius, where its tendon passes posterior to the radial styloid region en route to the base of the second metacarpal. Deep to these superficial planes, the radius and ulna frame the interosseous space, with the elbow articulation superiorly and the radiocarpal complex inferiorly providing clear bony landmarks for orientation. Relationships read cleanly. Palpation and strength testing of wrist extension often hinge on separating ECRL activity from brachioradialis and extensor carpi radialis brevis, and a medial look helps clarify why resisted wrist extension with radial deviation preferentially recruits ECRL. Proximal forearm pain labeled as lateral epicondylalgia frequently involves the common extensor origin and nearby radial nerve branches, and the close superficial layering in this region explains why symptoms can overlap between tendinopathy and radial tunnel irritation. Distally, the ECRL tendon’s course toward the second metacarpal makes it a common reference when teaching dorsal wrist anatomy and the extensor retinaculum compartments, and it also matters during tendon transfer planning in radial nerve palsy.
