Description
Shown from an anterior perspective through the radial forearm, the extensor carpi radialis brevis (ECRB) is identified deep to the more superficial extensor carpi radialis longus and extensor digitorum, tracking distally along the lateral aspect of the radius toward the dorsum of the wrist. Proximally, its muscular belly arises from the lateral epicondyle region of the humerus via the common extensor origin, lying lateral to the brachioradialis and just distal to the elbow joint. Distally, the tendon courses toward the base of the third metacarpal, passing beneath the extensor retinaculum in the second dorsal compartment with extensor carpi radialis longus, while the radius remains lateral and the ulna medial in anatomical position. Bones of the wrist (carpals), metacarpals, and phalanges provide clear bony landmarks for orientation. ECRB anatomy matters because this tendon is the usual pain generator in lateral epicondylitis (tennis elbow), where microtearing at the common extensor origin produces focal tenderness just anterior and distal to the lateral epicondyle. It is also a key structure when teaching wrist extension mechanics, because ECRB produces extension with less radial deviation than extensor carpi radialis longus, and its line of pull becomes clinically apparent during resisted wrist extension testing. Small muscle, common problem.
