Description
Running along the lateral forearm in anterior orientation, the extensor carpi radialis brevis (ECRB) is presented in relation to the humerus, radius, and ulna, with the elbow, radiocarpal joint, and the carpal, metacarpal, and phalangeal bones maintained for bony context. Proximally, its muscular belly lies deep to the more superficial extensor carpi radialis longus and brachioradialis region, arising from the lateral epicondyle of the humerus via the common extensor origin and blending with the lateral collateral ligament complex at the elbow. Distally, its tendon courses toward the dorsum of the wrist to insert on the dorsal base of the third metacarpal, placing it central and slightly medial to extensor carpi radialis longus at the level of the metacarpal bases. Deep. Precise. Clinical teaching often treats the lateral epicondyle as a single point of pain, but ECRB is the structure most often implicated in lateral epicondylitis (tennis elbow) because of repetitive wrist extension against resistance and force transmission through the common extensor tendon. That makes its proximal attachment and its relationship to the radiocapitellar region a practical map for examination maneuvers such as resisted wrist extension with the elbow extended, and for injection planning when conservative care fails. Surgical readers will also value the distinction between ECRB and adjacent radial-side muscles when debridement of degenerative tendon is discussed.
