Description
Running along the lateral (radial) margin of the forearm, the extensor carpi radialis longus is highlighted against the humerus, radius, and ulna in an anterior orientation. Proximally it arises from the lateral supracondylar ridge of the humerus, just superior to the lateral epicondyle, and then courses distally across the anterolateral elbow region. Its long tendon continues toward the dorsum of the wrist to insert on the base of the second metacarpal, lying lateral to extensor carpi radialis brevis and near the brachioradialis in the proximal forearm. Clear bony landmarks anchor it. This anterior presentation is the one most learners struggle to reconcile with function because extensor carpi radialis longus extends and abducts the wrist while its muscle belly is readily approached from the front and lateral side of the elbow. It also helps separate two frequent sources of lateral forearm pain: lateral epicondylalgia often involves extensor carpi radialis brevis at the lateral epicondyle, while extensor carpi radialis longus originates slightly more proximal on the supracondylar ridge and can be implicated in proximal tendinopathy or overuse in racquet sports. Knowing that offset matters during palpation and during surgical exposure of the lateral column of the distal humerus. Use this asset in upper limb anatomy teaching to map muscle origins and insertions onto palpable landmarks, and in orthopedics or sports medicine materials covering lateral elbow pain, tendon overuse, and wrist extension mechanics.
