Description
Viewed from the lateral aspect of a right upper limb, the extensor carpi radialis brevis lies in the superficial posterior compartment of the forearm, immediately medial to extensor carpi radialis longus and anterior to the bulk of extensor digitorum as it courses distally. Proximally, its muscular belly arises from the lateral epicondyle of the humerus via the common extensor origin, then narrows into a tendon that runs deep to the extensor retinaculum and aligns with the lateral border of the radius before inserting on the dorsal base of the third metacarpal. The humerus, radius (lateral) and ulna (medial) provide bony context from elbow to wrist, with the carpal, metacarpal, and phalangeal skeleton shown distally to orient the wrist extensors in relation to the hand. This lateral presentation matters because extensor carpi radialis brevis is the principal driver of wrist extension with radial deviation under load, and its origin at the lateral epicondyle places it at the center of lateral epicondylitis (tennis elbow), where microscopic tendinopathy commonly involves ECRB rather than the more superficial longus. The relationship to the lateral epicondyle and the radial head region also supports teaching of provocative maneuvers and targeted injection or surgical debridement planning, where accurate localization of the common extensor tendon complex is the difference between relief and persistent symptoms.
