Description
Arising from the lateral epicondyle of the humerus and the adjacent supinator crest, the extensor carpi radialis brevis (ECRB) occupies the superficial posterolateral compartment of the forearm, lying medial to extensor carpi radialis longus (ECRL) and anterior to extensor digitorum as the muscle bellies converge toward the dorsum of the wrist. From this lateral perspective, the common extensor tendon is seen proximally, with ECRB’s shorter tendon coursing deep to ECRL toward the base of the third metacarpal, while ECRL continues more anteriorly and distally to the base of the second metacarpal. The brachioradialis forms an anterolateral border near the elbow, and the lateral supracondylar ridge provides a bony reference superior to the extensor mass. Clear landmarks. ECRB matters because its proximal aponeurosis at the lateral epicondyle is the primary pain generator in lateral epicondylitis, and this lateral view aligns with palpation and ultrasound-guided injection trajectories aimed just distal and anterior to the epicondyle. The relationship between ECRB, ECRL, and the radial head region also frames the course of the deep branch of the radial nerve as it passes into the supinator, a common site of compression in posterior interosseous nerve syndrome that can mimic extensor tendon pathology. Distinguishing ECRB from ECRL is also practical when teaching wrist extension with radial deviation versus more neutral extension.
