Description
Viewed from the lateral aspect of the male hip, the gemellus superior appears as a short, strap-like muscle arising from the lateral surface of the ischial spine and running laterally toward the femur, just inferior to the piriformis. Its fibers converge with the tendon of obturator internus, with the gemellus inferior situated caudal to that tendon, creating the characteristic “triceps coxae” arrangement deep to gluteus maximus. Superiorly and anteriorly, the ilium and acetabulum frame the coxal socket, while the proximal femur is shown with a prosthetic femoral head seated at the acetabular component. The sacrum and lower lumbar vertebrae anchor the posterior pelvis in the background. Small muscle, tight neighborhood. For teaching deep gluteal anatomy, a lateral perspective clarifies how the gemellus superior contributes to lateral rotation of the extended thigh and stabilizes the femoral head in the acetabulum, acting in concert with obturator internus and the other short external rotators.
