Seen from a lateral perspective in the deep gluteal region, the gemellus inferior lies immediately inferior to the tendon of obturator internus and superior to quadratus femoris, spanning from the ischial tuberosity toward the medial surface of the greater trochanter. Its short, fleshy fibers blend with the obturator internus tendon as they course laterally and slightly anteriorly to reach the trochanteric fossa, forming part of the posterior hip’s short external rotator complex. Superficial landmarks such as the acetabulum and proximal femur frame the muscle’s relationship to the hip joint capsule. Small but exact. This angle matters because the gemellus inferior is often taught as a distinct muscle but functions mechanically as a component of the obturator internus-gemelli unit, producing external rotation of the femur and contributing to posterior hip stability during flexion. In posterior and posterolateral hip approaches, appreciating the layered relationship of obturator internus, gemelli, and quadratus femoris helps orient repair of the short external rotators and avoid iatrogenic injury to nearby neurovascular structures, including the sciatic nerve as it descends posterior to the hip. It is also a useful reference when correlating deep gluteal pain patterns with pathology in the obturator internus and adjacent tendons, which can mimic intra-articular hip disease.
