Description
Running vertically along the posterolateral cervical spine, the iliocostalis cervicis appears as a deep column of erector spinae fibers lying inferior to the mastoid region and superior to the upper thorax, with its slips tracking from the upper ribs toward the posterior tubercles of the mid cervical transverse processes. Superficial to it, the trapezius and deltoid contour the shoulder, while the sternocleidomastoid forms a strong anterolateral boundary from clavicle and manubrium to the mastoid process. Anteriorly, the masseter and adjacent muscles of facial expression frame the mandible and periorbital region, establishing clear surface orientation for a lateral neck dissection concept. Skin remains intact. A red vessel, rendered in standard arterial convention, courses in the superficial lateral neck for topographic reference. Iliocostalis cervicis often gets ignored in favor of sternocleidomastoid and trapezius, yet its fiber direction and segmental attachments matter when you are teaching extension and ipsilateral lateral flexion mechanics of the cervical spine as part of the erector spinae group. Palpation and dry needling targets commonly drift too anterior or too superficial, landing in levator scapulae or splenius instead of the deeper iliocostalis cervicis, and that distinction becomes clinically relevant in cervicogenic headache patterns and post whiplash myofascial pain. For posterior cervical approaches, the muscle’s position deep to the superficial layer helps explain why midline exposures spare it while more lateral dissections encounter it near the transverse processes. Rehabilitation medicine texts, manual therapy coursework, and surgical anatomy teaching sessions all benefit from this lateral, under-the-skin rendering to connect surface landmarks (mastoid, mandibular angle, shoulder contour) to the deep extensor compartment.
