Description
Seen in lateral profile, the longissimus cervicis runs as a slender erector spinae column along the posterolateral cervical spine, deep to the more superficial splenius capitis and posterior to the transverse processes of the mid cervical vertebrae. Its fibers arise from the transverse processes of the upper thoracic vertebrae (classically T1 to T5) and ascend to insert on the posterior tubercles of cervical transverse processes (commonly C2 to C6), placing it medial to iliocostalis and lateral to spinalis within the erector spinae group. Superiorly, the muscle approaches the suboccipital region beneath the skull base, while inferiorly it blends into the thoracic extensor mass near the first ribs and upper thoracic vertebrae. Intervertebral discs and facet levels are oriented clearly. A clean landmark set. Functionally, this view matters because longissimus cervicis contributes to cervical extension and ipsilateral lateral flexion, and it often becomes a pain generator in whiplash-associated disorders and chronic mechanical neck pain, where myofascial trigger points can refer discomfort toward the mastoid region and posterior auricle. Lateral orientation also clarifies its relationship to the cervical zygapophysial joints and transverse processes, which are key landmarks for posterior cervical approaches and for distinguishing deep extensor anatomy on MRI when evaluating paraspinal muscle atrophy after cervical fusion or radiculopathy.
